1. Title should indicate MS MISS MRS or MR.
2. Your name.
3. Designation. Use if you have a professional title only.
4. TYPE YOUR NAME EXACTLY AS YOU WANT IT TO APPEAR ON YOUR CERTIFICATE. We hope to present these to you at the reception.
5. Permanent address is where you want your mail sent. (Either Gainesville area or Hometown.)
6. Business Address if you are faculty.
7. Phone is your office or business phone number first, then home phone, and then fax number.
8. Email address.
9. Phi Kappa Phi Initiation date is April 24, 2007.
10. Fill in social security number and UF ID#.

Important Note

To receive a notation on your transcript that you are a member of Phi Kappa Phi, you MUST write your UF ID # on your application!

There is no blank for the UF ID #, so please write it at the top of the application before mailing it.

11. University Chapter Where Elected is the University of Florida.
12. Chapter number is 007.
13. Election status: Your status as of April 24, 2007.
14. Emblem desired: All emblems are the Phi Kappa Phi insignia. The adapter for use will determine if it will be a pin (clasp), charm (top hole for hanging), or tie tac (push pin/chain through the buttonhole style). We hope to be able to present these to you at the reception.
15. Publication Preference refers to the Phi Kappa Phi newsletter that you will be receiving while an active member.
16. Major.
17. Degree sought.
18. Proposed graduation date.
19. Your age.
20. Include check made payable to PHI KAPPA PHI for $50.00. Your emblem and certificate will be awarded to you at the reception.
21. Mail the completed forms and your check to:

    Please get these to her no later than April 20, 2007, so that you can be properly recognized at the reception.

SPECIAL NOTE: If you are responding after April 19th, you may expect that your membership will be processed with the submission for the next semester.

Don't forget the recognition reception to be held on April 24, 2007. Please print and fill out the RSVP form, and include it in the envelope with your application form and check. If you cannot attend the reception, your certificate and desired emblem will be mailed to your permanent address after the reception date.
